Conduct psychosocial assessments and reassessments in client's home including their social/emotional issues and practical arrangements related to the client's health;
Collaborate with the Nurse Care Manager in assessing, planning, providing initial and ongoing services for clients including the development of the client's individualized care plan including short and long-term goals;
Document client contacts, progress notes, and care plan goals on an ongoing and timely basis as the program follows a medical model of service delivery where every problem that is identified must be qualitatively addressed throughout the life of the client's case;
Consult with the Site Director on an on-going basis to review client's needs, progress, and other related matters requiring clarification and guidance;
Collaborate with community partners for additional services and support for the clients;
Participate in all California Department of Aging Utilization Reviews/Quality Assurance activities; and
Attend trainings as directed.
Requirements :
PATTERN I
Master's degree in Social Work or an approved two (2) year (60 unit) Master's degree in Counseling.
PATTERN II
Possession of a Master's Degree must be acquired within three (3) months of the application date for the position and prior to appointment.
